ISO 45002 requires an incident investigation to determine what happened, why it happened, and what can be done to prevent it from happening again. It specifically identifies the need to consider human behaviour, as well as wider systemic failures. This course explores:
- What are human factors and incidents?
- How can we model incidents?
- What behaviours cause incidents?
- Why do people make mistakes?
- What do we bring to incidents investigation?
- How do we add more value for investigations?
The course provides delegates with a range of tools and techniques that they can bring to incident investigation, to allow a greater insight into what provoked the incident, what can be done to prevent re-occurrence and how investigators might need to consider their own biases when investigating. The course will explore the importance of Just Culture and includes an interactive worked example using material developed by a large UK plc to explore some of the factors that will contribute to an incident. The course explores the importance of Just Culture and provides a flow chart to examine how the Reason/Rasmussen error model can be integrated into the COM-B model of behaviours to support effective investigation.
You will be able to apply an additional set of tools and techniques when you and your colleagues are undertaking accident investigations. You will be able to analyse the drivers for human behaviour and apply a bias model to support you in avoiding some of the common pitfalls associated with incident investigation.
